All Forms link or through one located at the top of every page. The placement of a Chinese Crested, wherein all the details of your situation aren't known, can result in the dog not working out in your home. Please be as thorough as possible in describing your home situation.
Your Prospective Adoption Application will automatically be submitted and will be forwarded to the Coordinator for your area. Make sure to include the correct E-mail address.

Crest-Care Inc. will make every effort to match a rescued dog to your needs, preferences and environment. Our goal is to have the Crested just as comfortable with you as you are with it. Our rescues come from a variety of backgrounds, so evaluation and placement can take a few weeks. Processing your application can average two weeks, but of course, may take longer so, please be patient. We are working in your behalf as well as the Crested's..
Sometimes we have more applications than Cresteds, or a rescue may need extensive vet care and recuperation or evaluation time. While there may be several dogs in foster care at the time you apply, it still may take awhile for the right one to become available. Therefore, it is wise to submit your application right away. If you wait until the rescue's picture is posted, there may be several approved applications ahead of yours and the dog could be placed very quickly. By gaining approval first, you'll improve your chances of getting the one you want as soon as it is ready to be adopted.
While all coordinators and reps try to reply to each inquiry, you may not hear from him/her until a match is found. Feel free to contact your coordinator/representative occasionally to reinforce your interest and/or advise of any changes in your initial application information.
When an appropriate match has been found, you will be contacted to further discuss your application, the particular Crested's characteristics, and to schedule a home visit, if possible.
Once your application is approved and the perfect match is found, an adoption contract must be completed and signed. At the time the contract is presented to the coordinator or representative, the adoption fee, see below schedule*, also must be paid in full. The fee is used to benefit other rescues, some of which may need more medical care or other treatment than your dog did.

As of May 15, 2009 our Adoption Fees will be increased.
Our new adoption fee will consist of a sliding scale based on the animals approximate age.
- Dogs determined to be under 1 year of age, adoption fee $350.
- Dogs determined to be between the ages of 1-3 years of age, adoption fee $300.
- Dogs determined to be between the ages of 4-6 years of age, adoption fee $275
- Dogs determined to be 7 years of age and older, adoption fee $225, but adoptions fees may vary according to region, medical expenses, and transportation.
Should you choose and be approved to adopt a Chinese Crested that is being fostered in a city not within driving distance of your home, you may arrange to have the dog shipped. These arrangements must be coordinated with the fostering rep that will be shipping to you. He or she may be able to assist you in shipping considerations. You also can visit the websites of airlines that serve your closest airport to obtain the regulations stipulated by each one. Shipping fees can range from approximately $150 to $300. You will be responsible for all shipping costs, including the crate.
After all arrangements are made, the contract completed and the adoption fee paid, the Chinese Crested will be placed with its new family.
Follow-up calls and/or e-mails will be made by your coordinator or representative to ensure that the Crested is adjusting well and things are running smoothly with it in your home. At this time, you may ask for any advice on care, training, habits, or any other concerns you may have in the early weeks of adjustment.
